Working cards are now being processed at the main office of the District Administration Building, located at 430 W. Washington Ave. By appointment only, staff are available to process working cards at the District Administration Building Monday through Thursday between 8:30 a.m. and Noon. by appointment. Please call 607.735.3000 to schedule an appointment. Student (s) must be accompanied by a parent and/or guardian. Proof of student age and proof of a physical within the past 12 months are required.

For students who do not have a copy of their physical:

Due to Covid-19, many Physician offices are not open for students and families to obtain copies of physical exams required for working cards. For many students, their school’s health office has their physical exam on file. A school nurse is available, by appointment only, to issue working cards for students who are unable to locate a copy of their physical. If you have a copy of your physical, please continue to call the Administration Building for an appointment at 607.735.3000.

Call the Ernie Davis Health Office at 735-3110 Mon.-Thurs. 9am-12pm to schedule an appointment with a school nurse. From there an appointment will be set up to obtain the working card. Please note if your child attends Elmira High School or Broadway Academy, it may take up to 24 hours for an appointment to be scheduled as the nurse will need to travel to those schools to look up your child’s medical record. Staff is limited during these difficult times.
